Membership & Terms of Agreement

• I understand that my participation in any Guided Spiritual Experience conducted as part of this Ministry is purely voluntary.
• During SoulRoot’s Guided Spiritual Experiences, I may voluntarily receive sacred plant medicines.
• I have been informed of each plant medicine’s effects as well as the objectives of taking it within the ritual led by an experienced Minister.
• I have sought SoulRoot’s Guided Spiritual Experience as a result of research and personal reports for the potential of a profound spiritual transformation.
• I understand that this Ministry makes no claim or promise regarding the curing of any illness or the nature of any spiritual experience.
• I understand that spiritual experiences are personal and sacred to each individual and that what may occur for one person, may not necessarily lead to the same experience in others.
• I understand that any spiritual practice in which I choose to participate may be physically, mentally, emotionally, or spiritually demanding.
• I accept full responsibility for anything that may occur in this Guided Spiritual Experience including disorientation and any and all possible manifestations of physical, emotional, or mental changes.
• I acknowledge that the risks and potential benefits of my participation have been explained to me and I freely choose to participate in SoulRoot’s Guided Spiritual Experiences accepting full responsibility for whatever may occur, anticipated or unanticipated.
• I hereby acknowledge and voluntarily assume the full risks of any physical or other injury, damage or losses, either to myself or caused to others by me during any spiritual Experience organized and guided by SoulRoot Ministry.
• I hereby waive the liability of and agree to hold harmless SoulRoot Ministry including all of its founders, members, associates, employees, agents, staff, successors, volunteers, other participants and any and all property owners of property where the Member’s Experience occurs. I further agree to defend and indemnify them from any claims, suits or demands.
• I understand that this agreement is binding upon me, my spouse, parents, agents, heirs, executors, and administrators.
• I agree that each and every provision of this agreement is independent of any other provision and may be enforced even if other provisions are not enforceable.
• I have read and understand this waiver, have had time to reflect upon it, and agree to this waiver voluntarily.

Health Disclosure and Screening

• I affirm that I have fully and honestly disclosed to SoulRoot Ministry any physical or mental health conditions, medications, or other substances that could interact with or be affected by participation in a Guided Spiritual Experience.
• I understand that SoulRoot Ministry is not a medical provider, does not perform medical screening, and relies entirely on my own disclosures in determining my suitability to participate.
• I understand it is my sole responsibility to consult with a qualified medical professional prior to participation if I have any health concerns, and that SoulRoot Ministry bears no responsibility for any consequence arising from an undisclosed or undiagnosed condition.

Legal Status Acknowledgment

• I understand that the legal status of sacred plant medicines offered within SoulRoot’s Guided Spiritual Experiences may vary by state, locality, and jurisdiction, and it is my own responsibility to be informed of and comply with the laws applicable to me.
• I am participating in this Ministry’s Guided Spiritual Experiences as an exercise of my own sincerely held spiritual beliefs, of my own free and informed choice, and not at the direction, suggestion, or inducement of SoulRoot Ministry.

Governing Law and Venue

• This Agreement shall be governed by and construed in accordance with the laws of the State of Washington, without regard to its conflict of law principles.
• Any dispute, claim, or controversy arising out of or relating to this Agreement or my participation in SoulRoot Ministry’s activities shall be resolved exclusively in the state or federal courts located in Washington State, and I consent to the personal jurisdiction of those courts.
